The new Glider™ from ASI Dental Specialties is described as a game changer when providing efficient dental implant and surgical procedures as an integrated over the patient dental surgical table. Dr Danny Domingue shows off the final result of an implant-supported prosthetic ear he designed and placed.

Patients with extreme oral health situations are common at the Lafayette, Louisiana, practice of Danny Domingue, DDS, FAAID, DABOI/ID, and through the use of technology, creativity, and his dedication to finding solutions, he is helping to change those patient’s lives.

A general dentist, Dr Domingue does much more than providing full arch restorations, but those complex cases were the focus of his mainstage presentation during the 2023 American Academy of Implant Dentistry (AAID) Annual Conference. While those cases can be complex, and each one requires some unique problem solving, Dr Domingue believes dentists have the tools necessary to deliver lasting solutions for those patients who need implant-retained full-arch restorations.

“It should not be stressful by any means,” he says.

While showing off his implant workflow, Dr Domingue highlighted a number of technologies that play important roles in delivering optimal outcomes to patients. These technologies include digital impressions, CBCT scans, extraoral facescans, virtual articulators, digital treatment planning, 3D printing, and photogrammetry. Still, he relies on analog impression materials to capture specific details when the depth of a socket or the presence of too much blood makes his Medit intraoral scanner unable to deliver the detail and accuracy he needs to design the case.

Still, that intraoral scanner is an essential part of the process as he employs digital impressions and extraoral facescans to work out the proper bite for the patient at the start of his workflow. He then doesn’t have to focus on the bite and occlusion while designing the rest of the case.

These images can then be used to create a digital waxup and a smile visualization to show patients their potential treatment outcomes. Dr Domingue highlighted the importance of letting patients see what can be done at the outset as this can get them invested in pursuing and completing treatment.

“People are very visual, they want to see something,” he says.

When planning his implant treatments he uses BlueSkyBio software to plan the implant locations and then to design a custom surgical guide. During the presentation, Dr Domnigue showed how it takes him just 12 minutes to design a custom guide for an edentulous patient. Designed in several parts, and linked together with pins and magnets, the guide can be 3D printed in his practice, and he says this makes placing the implants simple.

After the implants are placed, he uses photogrammetry to verify position for passive fit of the full arch restoration. This information is sent to the lab to complete restoration design, and he says overlaying the follow-up CBCT scan onto his planned treatment is satisfying as it lets him see the accuracy of his treatment.

“It’s really cool to see your implant positions overlapped to your preop positions, just like you planned,” he says.

After sharing his workflows and protocols, Dr Domingue discussed the importance of maintaining the gingiva even in full-arch restorations. He noted that for single implant cases, gingival management is a part of the workflow for most dentists, but it seems to be an afterthought when providing full-arch restorations.

The time for tissue management is while the patient is wearing the temporary restoration, he says. To ensure a healthy and proper gingival profile, Dr Domingue asks his lab to design the temporary to put some extra force on the gingiva which will help create a healthy gingival profile as the implants heal.

Another area where Dr Domingue says clinicians could be doing better is when it comes to designing hygienic full arch restorations. There should be no reason to remove a fixed full-arch implant restoration for a patient’s hygiene appointment, but many practices do this because the restorations have gaps or voids between the restoration and tissue that are difficult for patients to care for with the restoration fixed in place.

“I don’t take these out, because I don’t design them like that,” he says.

Dr Domingue says clinicians need to make sure not to design full arch restorations with cantilevers, or other unhygienic design features.

In the final segment of his presentation, Dr Domingue shared a pair of unique challenges he was able to solve with the same digital tools he uses with implant cases. In the first example, he shared the case of a patient who, along with needing dental treatment, was also missing an ear. Dr Domnigue demonstrated how he designed produced and surgically placed several implants with an attachment bar to hold the prosthetic ear in place in the same way fixed removable dentures are retained.

The second example focused on fowl patients, as a group of researchers studying duck behaviors came to him looking for ideas on how to make a better harness for the computers they attach to ducks and use for the research. Using the same technology he uses to design implant cases for his human patients, Dr Domingue walked through his process of developing a framework that can be affixed to a duck with implants and then provides a solid connection on which to mount the research equipment.

Dr Hogan: Hey I'm Dr Adam Hogan with the Atlantic implant Institute in Full Implant Choice.

I was here yesterday doing a full digital workshop for all-on-4 full arch immediate load fix treatment here at the AAID annual event and [we are] excited about implant dentistry and all the new innovations, all the new technology, it's just so exciting. It kind of keeps us young reinvigorates us, just the precision, the quickness to care, the ability to deliver care to more people. It's just super exciting. And then another thing is just the enthusiasm and the young doctors eyes, their eagerness to learn and discover new procedures and new ways of doing things. And I mean, I just I love bringing that to the new doctors and educating them and helping them on their on their journey. What are some of the key things that every doctor placing in place needs to be considering? How much time do we have? 

I have like days and days and days of content and all the things I would do doctor has to consider with implant dentistry. But you know, first and foremost, consider the people that you're treating, okay, delivering good quality care to those people. It's not about your implant number, your arch number, it's about making a difference in someone's life, and giving them the kind of care that hopefully will last a lifetime. And that's what every young doctor needs to take out of this education here at the AAID. Full arch dentistry is something that absolutely every doctor can attain in their practice, when they put in a little bit of work with the education and with the right products. It really revolves around digital workflow, which is what my workshop talked all about. It's impossible to deliver full arch care on a quality and with consistent levels of care in a timely manner. People doing full arch in an old analog workflow. Let's face it, it's just it's tedious. It's hard, they're gonna give up. I mean, I almost gave up. 

But the technological advances that make it more feasible for every doctor are amazing photogrammetry number one, just being able to take a quick scan and seconds, 30 seconds, 30 seconds max and be done with it, and know that it's accurate and repeatable. And that passive fit you get where you don't have restorations, braking, snapping emergencies that wake you up in the middle of night or on a Saturday bring you to the office. So photogrammetry number one, to facial scanning, being able to digitally plan a case to the face and be able to repeat it and just have it done in one just have your final set of teeth done. And one, the key frustration I hear from all doctors doing full arch is it just takes too much time. And it's so hard to make patients happy. And there's so many revisions, revisions is a dirty word you don't want them, you want to be done in one. And to do that you've got to have good facial scanning and proper eye cam photogrammetry. 

Yeah, I think that the public is becoming very aware of the possibility of full arch media care. Five years ago, I was inundated in my practice of full implant choice with patients messaging us just trolling us on the internet. This is impossible, you can't do this implants take two years. And now it's just common household names. You know, if all-on-4, you know, immediate teeth in a day people know about it. So they're searching it. I think that there's a lot of the public out there needing this care wanting this care. And so any doctor that wants to deliver it just really has to find out how to do it, take the education and put themselves out there and offer it and it'll be available. There's plenty of miles and arches to go around for everybody. The idea that this is this is a wonderful, absolutely wonderful event. I've been involved with the AAID since year one. My mentor Truman Baxter was a pioneer took the very first Maxi course placed implants since the hill Tatum days. So it's important to have a mentor. And you're not going to get a mentor without coming to an event like the AAID. You got the best mentors and all these hallways, the best education at the ad isn't on the main podium. It's not in my workshop. It's in these hallways where you can meet doctors, like myself, or hundreds of others here and get yourself a mentor that can carry you through this journey. And to AAID is great for credentialing. You owe it to yourself to get credentialed in the AAID so that you know that you you have a bona fide certified authority to provide this kind of care, but also the public knows that you do and the public can find you as a safe provider for their treatment. First up, Premier Dental recently announced the availability of Two Striper Perforated Diamond Finishing Strips. These finishing strips are designed to assist in debris removal during interproximal reduction, shaping and contouring. They come in two versions, one with an uncoated safe center for easy access through the contact point, and another with a serrated center to facilitate access in tight spaces between teeth. These handheld autoclavable stainless steel finishing instruments are available in various widths and grits to suit different clinical needs.

Denti.AI recently received FDA 510(k) for Denti.AI detect, an artificial intelligence-powered imaging solution that can analyze both intraoral and extraoral radiographs, including the industry's first clearance for AI analysis of panoramic images. The software is designed to identify common dental diseases, such as caries, periapical lesions and bone loss. Clinical Studies featuring the participation of more than two dozen dentists showed a 26% increase in treatment opportunities based on using Denti.AI detect. This new solution also integrates seamlessly with the company's Auto-Chart platform to simplify disease charting and treatment plan coding.

Finally, clear aligner solutions provider ClearCorrect has unveiled a series of innovations to expand orthodontic treatment options. These updates include enhancements to the Premier Aligner, Digital Workflow and practice growth offerings. Among the new clinical features are posterior bite ramps for cross bit cases, and an updated engager template featuring a thinner and more flexible material and a low flat trim line for more efficient and precise engager placement. ClearPilot 7.0 provides clinicians with more control over their treatment plans and includes features designed to make a more efficient treatment planning experience. It uses visualization tools like bite jump simulation, advanced editing tools for arch adjustment, additional ClearControl clinical feature controls, and user interface improvements to simplify building and evaluating treatment plans. Additionally, ClearCorrect is now offering a Practice Growth Package, which includes marketing, education, and business resources for clinicians looking to grow their practice and confidently treat a wider range of orthodontic patients.

Dental Professionals with a passion for implants descended upon Las Vegas this week to attend the 2023 American Academy of Implant Dentistry (AAID) Annual Conference.

With dentists, team members, and lab technicians from around the country and globe in attendance the 4-day meeting is featuring a wide array of in-depth and hands-on educational opportunities on topics such as implant technology and techniques, as well as dental sleep medicine, business, and more. But while the classrooms and exhibit hall have certainly been busy, the real action seems to be happening in between the events, as the hallways have been plenty busy as well with networking dental professionals.

While numerous topics were being discussed on the stages and throughout the Caesar’s Palace conference center hallways, some of the hottest topics included the digitalization of dental treatment planning, and the need for the most precise scanning technologies available, especially for full-arch cases, which were another hot topic at the event. While workflows and technologies powering single implant cases have become more streamlined and standardized, implant specialists throughout the show discussed the increasing demand for full-arch, implant-supported restorations.

While these restorations can make a huge impact on patients, there is growing concern about the long-term success of these restorations when they are not designed to properly manage the ways the patient’s bite will apply forces to the restoration. As these restorations are more common clinicians spoke about seeing an increase in broken screws and restorations.

To overcome this challenge, clinicians are adding photogrammetry solutions to their armamentariums. These systems use special cameras and specially marked scanning abutments to capture a highly precise record of the implant positions in 3 dimensions. While many practices use intraoral scanning and CBCT 3D imaging to accomplish this task, implant specialist are now saying more accuracy is needed to plan restorations with a true passive fit that can withstand long-term use by the patient.

In the exhibit hall, dubbed the Implant World Expo, implant and implant-adjacent dental companies were busy showing their latest innovations to dental professionals, and while education seems to be the focus for most attendees, the Expo always felt active and never felt overcrowded, even during the on-site lunch and Thursday night cocktail reception. Among the newer and notable innovations on display, and being discussed from the podium included Versah’s Densah Burs, the IMetric ICam4D photogrammetry system, and while it wasn’t specifically on display, several speakers mentioned Vortex implant.

Designed for a range of clinical applications, Densah burs are designed for osseodensification drilling. This means that instead of removing bone tissue while drilling like a standard bur, Densah burs are designed to redistribute the bone material within the socket, to provide less invasive surgical procedures and shorter healing times.

The ICam photogrammetry system has become a necessary piece of equipment for many clinicians specializing in full-arch implant cases. The solution provides greater precision for 3D positioning information than other technologies, while saving time for both the clinician and the dental lab when it comes to designing and placing full-arch restorations.

The Vortex screw was developed by Danny Domingue, DDS, FAAID, DABOI/ID, to overcome an issue with standard implant screws. While a standard screw is designed in a way that leaves small gaps beneath the screw and space between the wall of the restoration and the outside of the screw, Vortex screws are shaped with a tapered side and without a defined ledge beneath the screw head. This design aims to reduce forces that can break or damage full-arch implant restorations.

Patient engagement and communication solutions company RevenueWell has announced significant enhancements to its RevenueWell Marketing Platform as part of a new partnership with Synchrony and CareCredit

The latest updates are designed to allow dental practices to more effectively communicate CareCredit financing options to patients leveraging the full power of RevenueWell marketing campaigns, text messaging, and website templates. CareCredit’s health and wellness financial solution is part of Synchrony, a leading consumer finance company.

Additionally, new workflow enhancements for dental practices simplify the process of confirming and communicating pre-approvals, payment status, and pre-existing CareCredit accounts, further improving the ability to provide timely and accurate information about a patient’s payment and financing status.

• Personalized CareCredit URLs: By leveraging the latest CareCredit API, RevenueWell will take advantage of CareCredit personalized URLs (PURLs) for each patient, enabling CareCredit automation to pre-fill patient-specific information, and simplifying steps in the application process if someone has already been pre-approved or is an existing CareCredit card holder. With this process, the time required to fill an application is said to be significantly shortened for many patients, increasing the number of patients using CareCredit as a financing option and therefore the percentage of patients who can accept their treatment plan.

• Messenger Quick Links: Through enhancements to Messenger, practices can quickly send PURLs from within their real-time text conversations, providing just-in-time decision support for their patients navigating questions of payment and affordability.

• Campaign Merge Field and Filter: With the new enhancements, PURLs can be easily added to any campaigns, and dental practices can target an audience with relevant treatment options and promotions. Through RevenueWell’s advanced campaign features, users will now be able to include pre-approval as a targeting attribute alongside all other advanced targeting criteria such as treatment plan code. This is designed to enable practices to dramatically improve case acceptance by notifying patients with certain treatment plan codes that they are pre-approved for CareCredit financing.

• Off-the-shelf Content for Websites: RevenueWell’s ADA-endorsed web design services will now offer pre-designed and SEO-optimized web pages describing CareCredit services, enabling practices to showcase their financing options via any RevenueWell-designed website.

• CareCredit Payment, Pre-approval, and Payment Verification: These new enhancements make it easier to complete CareCredit workflows when the patient is in the office. Clicking on the patient’s pre-approved amount (or current cardholder) allows the practice to quickly accept or decline the pre-approval or process a payment. Another plus is that RevenueWell users can also easily check the status of a payment on behalf of a CareCredit user.

The new enhancements are included with most versions of RevenueWell Marketing Platform, including Professional and Premium editions, as well as any stand-alone implementations. With these changes, front-desk staff will no longer need to explain CareCredit options – the whole process will be managed on the patient side with minimal involvement from front desk staff. Common inquiries about pre-approvals and payment status can be handled quickly and easily with the new user interface enhancements.

In addition to improved efficiency, these latest updates are designed to improve case acceptance and patient satisfaction.

The main barrier to case acceptance is cost related to treatment. With these changes, dental patients will be more aware of their financing options, providing more opportunities for them to accept the care they need, reducing treatment backlog and increasing realized revenue, RevenueWell states.

According to a recent PYMNTS.com survey, patients are generally dissatisfied with their payment experience in healthcare—40% said they weren’t satisfied with their billing experience, 36% said they would switch providers if billing was too complex, and 27% said they would consider switching providers if they weren’t offered a payment plan or third-party financing. With these new enhancements, RevenueWell Marketing Platform reduces these potential patient satisfaction risks by simplifying the process and empowering patients with more options and more control.

The CareCredit enhancements were set to become available in the RevenueWell Marketing Platform on October 31 for all customers.

Pac-Dent, Inc., has unveiled an expansion of its line of Rodin™ 3D Resin Printing Materials with the launches of Rodin™ Sculpture 2.0 and Rodin™ Titan.

With these 2 new products, the manufacturer continues to push the boundaries of 3D-printed dental restorations through the continuous development and integration of cutting-edge ceramic nanohybrid technology.
