We May Have Found the World’s Worst Dental Practice

This practice might have you reaching for the closest bottle of hand sanitizer.

 earlier this week out of Moscow. According to the Sun, the photos were snapped by Julia Kalinina at the Russian dental clinic. Surprisingly, Kalinina told the doctors that she would be taking and sharing pictures of the clinic, the Sun reports. The doctors then gave her permission to continue.

“The dentists know I took the images,” the Sun quotes her as saying. “I said I’ll try to publish them and (the) dentists were glad. Authorities don’t pay attention to complaints.”

According to the Sun, there are two types of dental practices in Russia: private and public clinics. Public clinics, such as the one highlighted in the report, treat patients for free and rely on government grants.”

The Sun reports that Kalinina passed on treatment for tooth pain after seeing the clinic’s condition.

How Type 2 Diabetes Affects Oral Health in Children

A new study shows that children with Type 2 diabetes tend to have poorer oral health than children who don’t have the disease.

The study was published at the beginning of March in 

The study also looked at oral health outcomes for children with Type 2 diabetes who are also obese. It included three groups: 19 children at normal weight, 14 obese children, and 16 obese children with Type 2 diabetes. The children ranged in age from 10-19.

Researchers concluded that obese children with Type 2 diabetes tended to have poor oral health.

This catch may not have won an NFL game, but it did win our hearts. A video of a man saving one-year-old’s birthday cake went viral this week. The cake savior is — you guessed it – a dentist.

NASCAR announced its partnership with Aspen earlier this week in a

Authorities have charged a New York dentist in a crown-billing scheme.

WKBW, a Buffalo, New York-based ABC affiliate, reports

 that Michael Oros, D.D.S., has entered a plea of not guilty to grand larceny charges. Prosecutors say that Oros, 57, charged patients about $12,000 for high-end crowns, but gave them lower-quality crowns. Oros, authorities charge, then kept the difference.

WKBW reports that the alleged scam went on for nearly four years. Oros, whose practice is in Avon, New York, will be back in court in May.

The FBI has issued a cyber security alert that hackers are specifically attacking dental and medical facilities.

 (FTP) servers are the hackers’ target of choice, and that they are attempting to obtain patient data for the purposes of intimidating, harassing or blackmailing practice owners.

Under Attack: Dentistry Faces Growing Cybersecurity Threat

Highlighting a 2015 University of Michigan study, the FBI says that more than 1 million FTP servers currently allow anonymous access, potentially exposing protected patient data stored on servers. The FBI explains that anonymous FTP extensions allow anyone access with common usernames, such as “anonymous” or “ftp” without using a password.

Authorities also caution that hackers may attempt to use anonymous access to your FTP server to store malware, viruses, or to mount larger cyberattacks.

“In general, any misconfigured or unsecured server operating on a business network on which sensitive data is stored or processed exposes the business to data theft and compromise by cyber criminals,” the alert reads.

The FBI also issued suggestions on how dentists and other doctors can better protect themselves:

· Dentists should work with reputable IT companies.

· Dentists should have your contracted IT provider check to see if any FTP servers are operating in anonymous mode.

· If your dental practice has a legitimate need for your FTP to be operating in anonymous mode, then you should make sure no sensitive patient data is stored on the server.

The FBI says that hackers are attacking unsecure file transfer protocol systems in dental practices. Many of these servers, the FBI says, also contain sensitive patient information. The hackers are then using this information to blackmail dental practice owners, authorities say, noting that this type of breach can also lead to malware and viral cyberattacks.

FBI Issues Dental Patient Data Security Alert

, a 3D printer and printer material company, has announced the creation of a new 3D printing material for dentistry. The new material will be for printing indirect bonding trays, the company announced in a 

EnivisionTEC’s new E-IDB material will allow orthodontists to create flexible, custom trays, place orthodontic brackets more accurately, and place brackets that will release more easily, the company says. EnvisionTEC notes that four E-IDB trays can be built in about 45 minutes on the company’s Vida 3D printer. The new material, the company says, requires little post-processing.

According to Al Siblani, EnvisionTEC’s CEO, the new material cuts suppliers out of the equation.

“This change gives clinicians more control over their trays, and how tight or loose they prefer them, as well as the entire treatment process,” Siblani said in the news release.

In its news release, EnvisionTEC touted its integration with 3Shape. The 3D scanning and CAD/CAM software and hardware company recently made headlines of its own when it announced 

a wireless version of its popular Trios 3

Orthodontists stateside will be able to get their hands on the Trios 3 and E-IDB at the upcoming American Association of Orthodontists annual meeting, which takes place in San Diego from April 21-25.

Indirect bonding trays, according to EnvisionTEC, have certain advantages. They allow orthodontists to place brackets on teeth in half the time as the traditional, individual method. This process takes about two hours, according to the company.

Orthodonists using this system will create a 3D map of a patient’s mouth using an intraoral imaging device. They’ll then print this map, creating a tray. This enables them to determine more precise locations of a patient’s teeth. The orthodontist will then place brackets into the tray. The next step is to place the tray in the patient’s mouth, adhere the brackets, and then remove the guide, leaving the brackets in place.

EPA Rejects Anti-Fluoridation Group's Request, Is Commended by ADA

 The Environmental Protection Agency has rejected a group’s petition to ban fluoride from drinking water. The group had claimed that water fluoridation has a negative impact on citizens’ IQ. The EPA said that science does not support the outcome, noting that fluoridation has significantly reduced oral disease and tooth decay in communities with fluoridated water.

The DMD Top 5 for the Week Ending March 24

You don't have to be afraid of incorporating new technology into your practice. Check out these tips from Marty Jablow, D.M.D., from his lecture at the 2017 Hinman Meeting.

For the average dentist, the thought of embracing new technology produces a common reaction, according to 

In his continuing education session on Friday, March 24 at the 

, titled “What’s New In High Tech Dentistry,” Jablow provided attendees with information to make smart choices about the big-ticket items they purchase for their practices.

“You’ve got to think before you do stuff. In this case, you’ve got to think before you buy,” he said.

Put Your Personal Finances on the Right Path

Before delving into the specifics of the latest advancements in cone beam and carry detection technology, Jablow offered three pieces of advice:

 The newest entrants into the dental technology market might not be the best. These devices require time to reach perfection, he said. He also advised dentists to be realistic with their goals. You will not go paperless and switch to detail, X-rays within a month, he said.

 You need to include your team members in your tech acquisition decisions. “In many cases, you (the dental team) may be using the technology, not the doctors,” Jablow said. “Doc, give it up. Let the person who wants to run the project run it.”

You don’t need to have all the bells and whistles to have a high-tech image, he told Hinman attendees. But if your practice appears technologically sophisticated and features carefully selected high-end technology, it will translate to profit. Jablow gave the example of deciding to put an early LCD screen in an operatory. Patients thought this relatively low-cost piece of technology was cool, and the coolness factor helped bolster the practice’s image. “If nothing’s been changed in your office in 30 years, there’s a problem,” he said.

Jablow told Hinman attendees that you need to question your motives for purchasing the latest gadget or technology. Is it because you want a new toy, he asked rhetorically, or because you want to drive profit. Return on investment, Jablow said, should be a determining factor in every major equipment purchase. He added that sometimes dentists do want or need toys, too, but they should “know that’s the reason.”

Amid the myriad of technological advances available in dentistry today, Jablow took a deep dive into several that are most integral to the business of dentistry. The first topic he covered was Cone Beam imaging.

Thanks to the latest imaging advances, “we’re looking at the 3D world,” Jablow said. Just how much of that world you want to see, however, is an important consideration.

Because you are responsible for whatever you’re looking it in radiographs, Jablow asked, do you really need to be or want to be responsible for the entire skull? Cone beam imaging can provide you with just that, he said, but it can also provide you with more focused fields of view. You must ask yourself, “What do I want this machine to do for me?” Jablow said.

You also want to minimize radiation to your patients, he advised, which you can accomplish by narrowing your imaging field of focus. Providing further considerations on amounts of radiation, Jablow pointed out that your antiquated 2D imaging device could be blanketing patients with excessive radiation.

Another word of caution that Jablow provided with cone beam imaging: with the increase in detailed images comes an increase in responsibility. “If you are not absolutely sure, you must get the radiologist to read it,” he said.

You want a camera that suits your level of photography expertise, Jablow advised. He recommended the 

, which comes with many useful presets that can be enabled with the push of a button, reducing the learning curve of typical DSLR cameras. Also, he said, the Shofu camera can be wiped down. DSLRs might not stand up to harsh sterilization chemicals.

In addition to providing you with more detailed images that can assist in your diagnosis and with case acceptance, dental photography builds in another layer of liability protection. Jablow said he now photographs every tooth he puts a drill to better support treatment decisions. Photos, he said, when shown to peers or review boards will lead people to say, “Well, that’s not out of the realm of possibility.

The days of sharp explorer detection of dental caries are over, Jablow said. Though the method may have been useful 100 years ago when dentists lacked modern imaging, it often leads to false positives now.

Enhanced magnifigcation, such as illuminated dental loupes, are a must-have for the modern dentist. He likes 

, which come with three magnification settings.

“I would not let anyone in here treat me if they were not wearing some kind of magnification with illumination.”

What keeps dentists from making the big-ticket tech purchases they need to make their practices thrive? Fear. But they needn't be afraid. They just need to ask the right questions. This was the message that Marty Jablow, D.M.D., delivered to attendees of his 2017 Hinman Dental Meeting lecture, titled What's New in High Tech Dentistry.

Hinman Dental Meeting: Make the Right Tech Purchases for Your Practice

Authorities say that an alleged illegal California dentist also knew a good bit of chemistry. Police say the man was using his “unsanitary” illegal practice to work on teeth and make meth. Across the pond, children in the United Kingdom are facing an oral health crisis. And the University of Washington School of Dentistry is in a crisis of its own, this one of the financial variety.

ILLEGAL DENTIST HAD A SIDE GIG: MAKING METH

It was a different kind of dental lab. This one, authorities say, wasn’t producing prostheses, but meth.

 reports that Joseph Hirsch, of Redwood City, California, has been charged with practicing illegal dentistry “in an unsanitary office where he allegedly cooked meth.”

Hirsch is accused of possession and manufacturing of a controlled dangerous substance. He also allegedly had brass knuckles and ammunition, according to Fox News. Authorities aren’t sure if any patients were harmed by Hirsch.

Hirsch’s arrest was part of a three-month investigation, Fox News reports. When authorities served a warrant on the alleged illegal practice, they found an X-ray machine, dental instruments, the meth, and meth-making equipment.

It would seem that the United Kingdom’s children are in the midst of an oral health crisis.

 reports that tooth extractions among pre-school-aged children have increased 24 percent over the last ten years. Just last year, the Daily Mail reports, 47 infants less than a year old had a baby tooth removed.

The Daily Mail’s report indicates that parents who give their children bottles to lull them to sleep may be a part of the problem. Milk lactose residue accumulates on children’s teeth, and during sleep, salvia production slackens, leaving the sugary lactose there to do its damage, the report says. Constant breast feeding, the report indicates, can also be a contributing factor, noting that breast milk contains 7 grams of natural sugar per 100 ml.

Sugar-sweetened baby foods are also a problem. Marketed toward health-conscious but busy parents, many baby foods contain fruit juice for added sweetness, the report notes. One popular U.K. brand contains nearly 20 grams of sugar.

UNIVERSITY OF WASHINGTON SCHOOL OF DENTISTRY AWASH IN RED INK

The Seattle Times reports that the University of Washington School of Dentistry, which has been struggling financially in recent years, finds itself another $6 million in the hole. It’s debt load now totals $35 million, the newspaper says.

The school is under austerity measures to try to stanch the bleeding and the school’s dean, Joel Berg, has said a reorganization is in the works.

The Seattle Times’ report indicates that off the record, faculty members are saying the true money pit is the school’s Center for Pediatric Dentistry.

A dentist in West Des Moines, Iowa will pay a $10,000 fine over allegedly faulty dental implant work.

 reports that Steffany Mohan, D.D.S., was accused of incorrectly installing dental implants in several patients last year. The Iowa Dental Board commissioned an independent investigation into the complaints about her work on two patients. USA today says that this expert ruled that Mohan lacks “the clinical skills and knowledge for providing dental implant and denture prosthetics.”

When accused last May, the Register reports that Mohan denied any wrongdoing and said that she had never been accused of any poor work in her 20 years of practice.

A report released by the state dental board shows that Mohan did not admit to doing poor work. She will pay the $10,000 fine, be retrained and retested before she can practice again.

The 2017 Hinman Dental Meeting gave dentists the opportunity to put their personal finances on the right track with a continuing education session titled, "Achieving Financial Independence."

How would you like to be financially independent in 10 years?

John K. McGill (J.D., C.P.A, M.B.A.) delivered the information dentists need to achieve that goal at his continuing educations session, “Achieving Financial Independence,” at the 

“Believe it or not, most doctors are not wealthy,” McGill said during his morning session on Thursday, March 23.

According to McGill, only 5 percent of dentists can retire at 65 and maintain their current standard of living. He’s seeing a megatrend in the personal finances of dentists.

“Seventy is the new 65 when it comes to retirement age in dentistry,” McGill said.

What’s driving this trend? According to McGill, high practice overhead, large debt loads, unwieldy tax burdens, lavish lifestyle, lack of savings and poor investing are all to blame.

“Most doctors in here are overpaying their taxes by $10-20 thousand dollars per year,” McGill said.

He described working with doctors who were grossing $300 thousand or more annually who had little to no savings. The most well-paid dentist he ever worked with was bringing in $415 thousand per year, had no savings. He had been divorced three times, was paying three mortgages, maintained three country club memberships and was fond of Italian sports cars.

“Believe it or not, higher spending will not lead to more happiness,” McGill said. It only leads to more stress. Saving for retirement, he said should take precedence.

Just how much you need to retire, McGill explained, depends on several factors, including longevity, tax rates, investment returns and your spending level. Tax rates, he explained, may go up. Investment returns could be lower than you expected. The only thing you can control, McGill said, is how much you spend.

Would you treat a patient without taking their history or imaging, McGill asked rhetorically? You shouldn’t do the same regarding your personal finances.

“If you don’t know what your spending is, you’re committing malpractice on your finances,” he said.

Most people who track their money discover they were spending more than they thought. An internal study of his clients revealed that most believed they were spending $10 thousand per month, but they were spending $16 thousand. To reduce spending, McGill suggested the following:

· Work to make your adult children financially independent.

· Reduce the amount of money you’re pumping into their education.

· Reduce insurance costs. Nobody should be spending more than $7 thousand on life, disability, and business overhead insurance, he said.

After getting a handle on their spending, McGill said dentists need to identify savings opportunities. Typically, he said he’s able to find between $50-60 thousand in savings for his clients. Dentists, he said should focus on these savings opportunities:

Think of your HSA as tax-deductible retirement funding. When money is withdrawn from these accounts, it’s tax free up until the amount of your medical expenses. “That’s the best deal you’re going to get on earth,” McGill said. Properly funded, an HSA could contain as much as $500 thousand on retirement.

If you’re in a higher tax bracket, you’ll want to maximize your tax-deductible savings. But if you’re in a lower tax bracket, you’ll want to prioritize paying of your debt. Higher-earning dentists, however, should pay off their debt at the minimum rate, then when they sell their practice or retire, pay off their debt in one fell swoop, McGill said. Many younger docs, he said, will try to pay off their student loans first instead of saving for retirement, but this is not the best approach. It’s better, McGill said, to set aside money on a tax-deductible basis.

Automate your savings process by scheduling recurring deposits into your retirement accounts, McGill suggested. He’s seen this approach lead to higher amounts of net savings, because psychologically, you think of the automatically deposited money as already gone. Additionally, this strategy reduces the temptation to try and time the market with your investments. “Buying high and selling low won’t get you there,” McGill said.

 “You don’t have to have money sitting around for an emergency, you just have to have access to money for an emergency,” McGill said, noting that the notion of having six months of income on hand in a checking account is not the best approach. It’s better to have the money invested. Emergency fund options include home equity lines of credit, Roth IRAs and HSAs, McGill said. He added that Roth IRAs have certain tax advantages. He gave the example of a hypothetical investment of $50 thousand. If you received $10 thousand in returns on that investment, you can withdraw up to $50 thousand tax free. Cash, he said, should be a last resort.

This should be a priority, McGill said, once you’ve earned enough money to self-insure. For example, he said, if you’re Warren Buffet and have a net worth of $3 billion, do you really need to be pumping your money into a life insurance policy? Once you reach your financial goals, McGill said, drop your life and disability insurance policies. You don’t need them anymore.

 program, which allows drivers to take in that new-car smell on a monthly basis.

For a mere $1,500 each month, drivers can select a new car to drive without signing a lease or obtaining financing. They also won’t be on the hook for any repairs.

Dentists who appreciate fine, American automobiles can select their new monthly ride at the push of a button by using the company’s app. According to a news release from Cadillac, drivers can pick from the most well-trimmed models. A “white-glove concierge” will deliver their new selection to the location of their choice, and then drive away the old vehicle.

Membership entitles you to access to the “Platinum Level Trim” versions of the XT5, CT6 and V Series.

The program offers month-to-month membership, with no monthly commitments. The monthly fee covers registration, taxes and insurance costs, as well as maintenance. Mileage is unlimited. 

 reports that as of right now, the service is only available in the New York metropolitan area.

Providing more background on BOOK, Automotive News reports that the program might make having a car more appealing to city dwellers, who often must fight for parking and have readily available public transportation options. According to Automotive News, Audi ran a similar pilot program in Berlin in 2014.
